A new user login account request must come from the department manager or human resources and be signed . 

It must have the following information or cannot be processed

Full Name: 

Email address or Cellphone number that tickets will be received on:

If Cell phone, Carrier i.e. Verizon, AT&T, etc.:

Departments:  example: Engineering

Role:        example: Manager or basic user
